Our supported living services include supported accommodation, visiting and live-in care for the following client groups:
Dementia (1)

Dementia

Our dedicated group of staff are trained to gain better understanding of what it is like to live with dementia as well as support someone with the condition to live well. We cover topics such as identity, changes in behaviour, and the practical impact of dementia on the individual and carer.
At Sofeneth Care, we acknowledge the importance of person-centred care and focus on promoting wellbeing and needs of our clients. We also embrace a collaborative approach and promote a family centred approach.
With a healthy relationship being principal to improving the quality of life for clients with dementia, we strive to maintain professional therapeutic relationships in our quest to promote recovery.

Learning disabilities or autistic spectrum disorder

Sofeneth Care aims to provide safe, compassionate, and informed care. Promoting dignity, autonomy and maintaining equality is our benchmark. Delivering recovery-oriented care to clients with learning disabilities or autistic spectrum disorder is principal in achieving our objectives.
In line with the CQC requirement for registered providers, all our staff have completed The Oliver McGowan Mandatory Training on Learning Disability and Autism that is appropriate to their role.

Mental Health

Sofeneth prides itself on its reputation first choice when it comes to complex mental health care. We facilitate a highly personalised care experience with one-to-one support where needed and significant focus on enhancing daily living skills.
Sofeneth is able to meet the needs of almost all complex mental health diagnoses and is able to provide a short term or permanent home to residents who are looking to take a step towards community and independent living with access to plenty of local amenities such as leisure centres, parks, shops and job/volunteer opportunities.

Older People

We are committed to delivering safe, effective and quality person-centred care that ensures improved outcomes for our clients. Sofeneth Care is devoted to providing support that meets the mental and physical needs of older people.
We provide respite care and long/short term support. Led by our nursing specialists, care for the elderly involves dedicated support for an older person who wishes to continue living in their own home.
Sofeneth Care takes pride in tailoring care needs to meet the needs of individuals. We acknowledge that many older adults are burdened by sensory and/or cognitive limitations that limit their ability to hear, see, and/or understand their medical circumstances. Our staff are trained to deliver personalised care which facilitates conveying information in a manner that is accessible to each client.

In promoting dignity, autonomy and equality, we never use a one-size-fits-all approach in delivering care to our clients. We understand that everyone’s circumstances, conditions and preferences are going to be different. We believe that living with a physical disability should not prevent anyone from leading a full and active life. Whether an individual’s physical disability is recently diagnosed or is something they’ve lived with for many years, our care and support are designed to ensure they can live as independently as possible.
We embrace the underlying ethos of a collaborative approach; working in partnership with individuals and their families. Our approach to physical disabilities support is to place client at the centre of the support plan, and work with them to understand their needs and preferences, as well as their goals and ambitions.

Younger Adults (1)

Younger Adults

Our dedicated team of staff provide support to promote independent living. Options provided by Sofeneth Healthcare include person living in their own home as a tenant, rather than a resident, which could be a flat, house or a bedroom in a share house.
In promoting social inclusion, our aim is to enable them to live as they choose, make their own decisions about their day-to-day activities and their home while being part of a community. The accommodation can be adapted according to personal needs, for example with special equipment if the person has a disability.
Our specialist staff are trained to deliver personalised care that meet the specific needs of the individual, such as:
• Personal care
• Cooking, cleaning and shopping
• Managing finances and budgeting
• Finding employment /voluntary work and accessing education
• Establishing new relationships
• Building and maintaining social links with family and friends
